Pr\u00edlohou k \u017eiadosti je k\u00f3pia dokladu o zmene trval\u00e9ho pobytu alebo o nahl\u00e1sen\u00ed prechodn\u00e9ho pobytu a k\u00f3pia pracovnej zmluvy. Pr\u00edspevok sa neposkytuje, ak je zamestnancovi poskytovan\u00fd pr\u00edspevok pod\u013ea \u00a7 53.<\/li><li>Zmena pobytu na \u00fa\u010dely poskytnutia pr\u00edspevku je zmena miesta trval\u00e9ho pobytu alebo nahl\u00e1senie prechodn\u00e9ho pobytu, ak je miesto nov\u00e9ho trval\u00e9ho pobytu na \u00fazem\u00ed Slovenskej republiky vzdialen\u00e9 od miesta p\u00f4vodn\u00e9ho trval\u00e9ho pobytu najmenej 70 km alebo ak je miesto prechodn\u00e9ho pobytu na \u00fazem\u00ed Slovenskej republiky vzdialen\u00e9 od miesta trval\u00e9ho pobytu najmenej 70 km.<\/li><li>V\u00fddavky na b\u00fdvanie, s\u00favisiace so zmenou pobytu na \u00fa\u010dely poskytnutia pr\u00edspevku, s\u00fa preuk\u00e1zan\u00e9 mesa\u010dn\u00e9 v\u00fddavky na\n<ol><li>\u00fahradu za plnenia, poskytovan\u00e9 s u\u017e\u00edvan\u00edm bytu alebo<\/li><li>n\u00e1jomn\u00e9.<\/li><\/ol>\n<\/li><li>Pr\u00edspevok sa poskytuje najviac po\u010das \u0161iestich mesiacov. Zamestnancovi, ktor\u00fd bol pred z\u00edskan\u00edm zamestnania znev\u00fdhodnen\u00fdm uch\u00e1dza\u010dom o zamestnanie, sa pr\u00edspevok poskytuje aj po\u010das \u010fal\u0161\u00edch, najviac \u0161iestich mesiacov.
